Simply copy the PUP package to a FAT32 USB inside PS4 > UPDATE directory. Plug this USB into the console, go to Settings > System Software Update and follow the onscreen instructions.

In case of full re-installation, download the full PUP image and copy it to [USB drive] > PS4 > UPDATE. Completely turn off your console by holding the Power button for 7 secs then reboot after plugging the USB. In safe mode, select ‘Initialize PS4 (Reinstall System Software)’ and follow the onscreen instructions.
